Geriatric Needs Assessment and Management
Performed by a geriatric nurse practitioner and clinical specialist in the field of gerontology, our assessment and management service offers a comprehensive evaluation of a senior’s situation to help them, their families and heath care team determine his or her immediate and future needs.
The evaluation will identify current and potential problems, and, when feasible, recommend a range of safe solutions designed to help the senior remain home or return home to improve his or her quality of life and overall well-being.
A holistic and comprehensive approach will provide the following:
- Physical and medical review
- Home safety evaluation
- Fall risk assessment
- Cognitive and functional assessment
- Counsel in areas such as end of life decisions, power of attorney, health care proxy, and insurance
In addition, our geriatric care manager will serve as a liaison between seniors and their families when distance makes it difficult for them to address issues or problems and coordinate care or resolution. This is an invaluable service that enables seniors and their loved ones to be directly involved in the planning and implementation of services provided by Home With You.
